Appointment Booking & Cancellation

Booking: Appointments can be scheduled online, by phone, or via text (928) 955-0016 .
24‑Hour Cancellation Notice: Please provide at least 24 hours' notice to avoid charges. Cancellations or no-shows within 24 hours will be charged 100% of the service fee.
Late Arrivals: Arriving more than 10 minutes late may shorten your session or require rescheduling, with the full session fee still applying.

Packages & Gift Certificates

Packages and gift certificates are non-refundable, but transferable.
Gift Cards Expire 2 Years after Purchase, and packages expire 1 year after purchase.

Minors Policy

Clients under 18 years of age must be accompanied by a parent or legal guardian and have written consent before receiving any service.
For their safety, minors may not participate in OxyBoost Oxygen Compression Therapy.
We reserve the right to deny services to ensure the health and safety of each person.

Health Disclaimer

Clients must be in good physical health to receive services at Revive Wellness Recovery. It is your responsibility to consult with your physician before participating in massage therapy, cupping, IV therapy, or any other wellness services.
We reserve the right to decline service if we believe it may not be safe for your current condition.

General Liability Waiver

By booking and participating in services at Revive Wellness Recovery, you agree that you are doing so voluntarily and at your own risk. You release Revive Wellness Recovery, its staff, and affiliates from all liability for any injury, loss, or damages resulting from services provided.
Your participation constitutes acknowledgment and acceptance of this waiver.

Refunds & Rescheduling

All sales are final. Refunds will not be issued for missed appointments or prepaid services.
Prepaid sessions may be transferred to another individual upon request.

Privacy & Data Use

We are committed to protecting your privacy. Personal information collected during intake and booking is used solely for service delivery and is never sold or shared. All client records are confidential and securely stored.
