Appointment Booking & Cancellation Policy

Booking: Appointments can be easily scheduled online, by phone, or via text at (928) 955-0016. 

24-Hour Cancellation Notice: To avoid charges, please provide at least 24 hours' notice for cancellations. Our cancellation policy states that cancellations or no-shows within 24 hours will incur a charge of 100% of the service fee.

Late Arrivals: If you arrive more than 10 minutes late, your session may be shortened or require rescheduling, with the full session fee still applicable.

Packages & Gift Certificates

Packages and gift certificates are non-refundable but are transferable. Please note that gift cards expire 2 years after purchase, while packages expire 1 year after purchase.

Minors Policy

Clients under 18 years of age must be accompanied by a parent or legal guardian and have written consent to receive any wellness services. For their safety, minors are not permitted to participate in OxyBoost Oxygen Compression Therapy. We reserve the right to deny services to ensure the health and safety of each individual.

Health Disclaimer

Clients must be in good physical health to receive services at Revive Wellness Recovery. It is important to consult with your physician before participating in any wellness services, including massage therapy, cupping, or IV therapy. We reserve the right to decline service if we believe it may not be safe for your current condition.

General Liability Waiver

By booking and participating in services at Revive Wellness Recovery, you acknowledge that you are doing so voluntarily and at your own risk. You release Revive Wellness Recovery, its staff, and affiliates from all liability for any injury, loss, or damages resulting from the services provided. Your participation constitutes acknowledgment and acceptance of this waiver.

Refunds & Rescheduling

All sales are final. Our cancellation policy states that refunds will not be issued for missed appointments or prepaid services. However, prepaid sessions may be transferred to another individual upon request.

Privacy & Data Use

We are committed to protecting your privacy. Personal information collected during intake and appointment booking is used solely for service delivery and is never sold or shared. All client records are confidential and securely stored.
